1) you were employed in California by Deloitte & Touche LLP or Deloitte Tax LLP in its LEAD TAX practice line as a TAX ASSOCIATE or a TAX SENIORon or anytime after December 11, 2002OR;
2) you were employed in California by Deloitte & Touche LLP or Deloitte Tax LLP in its MULTISTATE TAX practice line as a TAX ASSOCIATE or a TAX SENIORon or anytime after November 4, 2004 and;
3) you were paid on a salaried basis and;
4) you worked in excess of eight (8) hours per day and/or in excess of forty (40) hours in one or more workweeks.
